The purpose of this study is to clarify the relevance of breastfeeding and its preventive effect on maternal hypertension as well as to evaluate the theoretical mechanism behind of it through systematic evaluation of existing articles.
For systematic evaluation of literatures in recent 5 years, 5 most suitable articles were selected with the key words, (breastfeeding or breastfeed or lactation) AND (hypertension or high blood pressure or hypertensive disorders) from PubMed, EMBASE, and Cochran Library, and carefully reviewed by 2 researchers.
Breastfeeding women have less frequently developed hypertension in their later life. Depending on the duration of breastfeeding, compared to nonbreastfeeding women, breastfeeding women’s odds ratio for developing hypertension are 0.87 (95% confidence interval [CI], 0.76–0.99), 0.83 (95% CI, 0.68– 1.00), and 0.79 (95% CI, 0.65–0.97) each for 0–6 months, 6–12 months, and greater than 12 months of breastfeeding. As the number of breastfeeding children increases, the incidence of maternal hypertension decreases. In addition, both partial and exclusive breastfeeding lower the risk of developing maternal hypertension. Though the mechanism of prophylactic effect of breastfeeding on hypertension is not conclusive, reset hypothesis, oxytocin release, the increase of ghrelin and protein peptide YY, as well as epigenetic programming are considered to be relevant to the etiology of the condition.
Breastfeeding prevents maternal hypertension later in life. Studies show dose-response relationship of breastfeeding as the duration matters. In addition, both partial and exclusive breastfeeding have preventive effect on maternal hypertension. Numerous mechanisms are continuously being reported and further studies are needed for clarification.

모유수유는 모유수유아를 위하여 다음과 같은 여러 장점들이 있다. 호흡기감염, 중이염, 비특이적 위장관감염, 궤사성장염, 영아 돌연사, 영아사망, 알레르기질환, 염증성 장질환(inflammatory bowel disease), 당뇨병, 비만, 소아 백혈병, 림프종, 신경 발달 이상을 감소시킨다(
최근 모유수유가 모유수유부 자신에게도 당뇨병, 심혈관질환과 같은 대사질환을 예방한다는 보고들이 있다 (
2014년부터 2019년까지 최근 5년간 발표된 문헌을 체계적으로 고찰하기 위하여 PubMed, EMBASE, and Cochran Library에서 “(breastfeeding or breastfeed or lactation) AND (hypertension or high blood pressure or hypertensive disorders)”의 키워드를 가지고 문헌을 탐색하였다. 총 7백 87건의 관련문헌이 있었다. 2인의 연구자에 의해서 이들 중 가장 적합한 문헌 5개를 골라 고찰하였다.
이번 논문의 체계적 고찰을 위해 포함된 논문은 2014년 이후, 최근 5년 이내에 쓰인 원저 논문과 메타분석 논문들 중 모유수유와 모체의 고혈압 발생 간의 통계학적 연관성과 양-반응 관계(예, 모유수유 기간 또는 모유수유아의 수와 모체의 고혈압 예방의 연관성)가 포함된 것들로 하였다. 특히,
그리고 0.79 (95% CI, 0.65–0.97)로 나타났다. 또 다른 단면연구에서는 국민건강영양조사(Korean National Health and Nutrition Examination Survey, KNHANES)의 자료에서 2010–2011년 50세 이상의 비흡연자이며 출산 경험이 있는 여성 3,119명을 대상으로 모유수유 자녀수가 가장 많은 5분위(5–11명)군과 모유수유 기간이 가장 긴 5분위(96–324개월)군을 가장 작은 5분위군과 비교 시 고혈압의 승산비는 각각 0.49 (95% CI, 0.31–0.75), 0.55 (95% CI, 0.37–0.82)로 나타났다. 또한, 모유수유아의 숫자에 따라 의미 있는 선형적 관계로 승산비 0.90 (95% CI, 0.83–0.97)을 나타내었다.
미국 내 흑인여성 21–69세 59,001명을 대상으로 한 연구에서 40–49세군에서 모유수유가 고혈압을 8% 감소시키는 것으로 나타냈으며 승산비는 0.92 (95% CI, 0.85–0.99)였다. 또한, 덴마크의 국가 출생 코호트에서 단태아출산 후 7년째에 추적된 63,260명의 여성들을 대상으로 했을 때, 4개월 이상 모유수유가 4개월 미만보다 고혈압 위험률을 2%–30% 감소시키며, 2개월 초과 부분수유도 2개월 이하 부분수유보다 10%–25% 낮은 위험률을 보였다.
마지막으로 메타분석 결과로 7개의 문헌에서 444,759명을 대상으로 하였을 때 고혈압 발생의 풀드 오즈비(pooled odd ratio)는 모유수유 기간에 따라 모유수유하지 않았던군과 비교 시 >0–6개월, >6–12개월, 그리고 >12개월에 각각 0.92 (95% CI, 0.88–0.96), 0.89 (95% CI, 0.86–0.92) 그리고 0.88 (95% CI, 0.84–0.93)을 나타내었다.
모유수유가 모체의 고혈압 발생에 관하여 예방적 효과가 있는지 여부와 관련된 최근 5년 내에 발표된 논문들을 체계적으로 고찰한 결과는 모유수유를 했던 여성들이 생애 후반에 고혈압 발생 위험이 덜하다는 것이다. 덴마크의 국가 출생 코호트에서 단태아 출산 후 7년째에 추적된 63,260명의 여성들을 대상으로 했을 때, 4개월 이상 모유수유가 4개월 미만보다 고혈압 위험률을 2%–30% 감소시키며, 2개월 초과 부분수유도 2개월 이하 부분수유보다 10%–25% 낮은 위험률을 보였다(

모유수유가 고혈압 예방에 미치는 효과와 관련하여 명확한 기전은 알지 못한다. 하지만. 관련된 동물실험과 사람을 대상으로 진행한 연구들에서 그 실마리를 찾을 수 있다. 첫 번째는 리셋 가설(reset hypothesis)로 임신 중 축적된 지방이 임신을 위해서 그리고 모유수유 시 에너지 공급을 위해서 쓰여야 하지만 모유수유를 하지 않은 경우 지방이 그대로 남지만, 모유수유를 하게 되면 리셋이 된다는 것이다(
두번째는 모유수유 시 분비되는 호르몬 옥시토신의 역할에 주목하고 있다. 동물실험으로 rat에게 옥시토신 주입시 장기적으로 혈압이 감소되었다(
세번째로 호르몬 그렐린(ghrelin)과 펩티드 YY (peptide YY, PYY) 또한 모유수유 여부에 따라 그 수치가 변화하고 고혈압 예방과 연관이 있다(
마지막으로 관련된 기전으로 최근 주목 받는 것은 모유수유에 의해서 모체가 후성유전학적으로 프로그래밍(epigenetic programming)된다는 것이다.
이 연구의 강점은 모유수유의 효과 중 모유수유아에 비교해서 상대적으로 덜 연구되어 있는 모체에서의 긍정적인 효과에 대한 연구이며, 최근에 발표된 문헌들을 중심으로 모유수유가 모체의 고혈압을 예방한다는 근거들을 근거체계(hierarchy of evidence)에 따라서 단면연구, 코호트연구 그리고 메타분석을 포함하여 체계적으로 분석하였다는 점이다. 그리고 모유수유의 기간에 따라서 고혈압 예방효과를 선형적 관계(linear association)로 양-반응적인 결과를 보여주었다는 것이다. 또한, 모유수유가 모체의 고혈압예방과 관련된 기전에 관하여 밝혀진 연구 결과에 따라서 동물실험과 사람을 대상으로 한 데이터에 근거하여 도식적인 기술을 하였다는 것이다.
이 연구의 단점으로는 모든 체계적 문헌고찰이 가지고 있는 한계처럼 고찰한 문헌의 양과 질적 수준에 의해서 영향을 받는다는 것이다. 예를 들면, 긍정적 결과들의 연구만 출판이 잘되는 출판편향을 배제할 수 없는 한계와 사람을 대상으로 하는 연구로 윤리적 이유로 무작위 대조군 연구가 아직 발표되지 않은 한계가 있다. 그럼에도 불구하고 저자들이 수행한 모유수유가 모체의 고혈압예방에 미치는 효과에 대한 체계적 문헌고찰 결과와 관련된 이론적 기전에 관한 동물실험 및 사람에서의 연구 결과는 상당한 근거들로 모유수유가 생애 후기에 모체의 고혈압을 예방한다는 것이다. 향후 보다 많은 연구들에 의해서 우리가 지지하는 결론은 보다 공고히 될 것이고 관련 기전도 명확해질 것이다.
이 연구는 최근 보고된 문헌들을 토대로 모유수유와 모체 고혈압의 관련성과 그 기전을 파악하기 위해 수행되었다. 이 연구 결과에서 모유수유를 했던 여성들이 생애 후반에 고혈압 발생 위험이 덜하고, 모유수유의 기간이 길수록 고혈압 위험도는 감소함을 확인하였다. 또한 부분모유수유와 완전모유수유의 경우 모두 고혈압 발생률을 낮추는 것으로 나타났다. 모유수유가 모체의 고혈압 예방에 관여하는 기전을 밝히기 위하여 시행된 동물 실험에서 리셋 가설(reset hypothesis)은 임신 중 축적된 지방(fat)이 모유수유 시 에너지 공급을 위해 쓰여 fat이 남지 않고 리셋이 된다고 하며, 사람에서 모유수유여성의 기초대사율이 높고, ghrelin 수치가 높아 음식 섭취 감소 및 체중 감소 효과가 있어 최종적으로 고혈압 예방 효과가 있음을 알 수 있다.
이 연구 결과는 생애 후반기 고혈압 예방을 위해 출산 후 모유수유의 중요성을 강조한다. 모유수유가 모유수유아를 위한 다양한 장점뿐만 아니라 모유수유부에게도 고혈압의 예방 효과가 있다는 결론이 그 기전과 더불어 공고해질 수 있도록 보다 심도 있는 연구가 필요하다.
