Trans Abstract

According to the World Health Organization (WHO), the health indicators and nutritional status of North Korean children have improved steadily since 2010. However, malnutrition remains a serious condition, with 19.1% of children stunted. in addition, although the nutritional status of North Korean refugee children has gradually improved since they settled in South Korea, a rapid increase in obesity rates has put them under a double burden of malnutrition and obesity. For the health of North Korean children who are at risk because of the recent rapid increase in coronavirus disease 2019 cases in North Korea, maternal and child health experts should work together to request immediate humanitarian assistance from the international community and the South Korean government. Furthermore, it is necessary to develop a maternal and child health program in preparation for the resumption of inter-Korean exchanges and cooperation in the future.

서 론

1990년대 중반부터 지속된 자연재해 등으로 인해 시작된 북한의 식량난은 최근까지도 여전히 심각한 상황이다. 북한당국은 “2030 의제 이행에 관한 보고서(Democratic People's Republic of Korea Voluntary National Review on the Implementation of the 2030 Agenda, 이하 ‘ VNR 보고서’)”에서 2020년에도 곡물수확량이 목표치 700만톤 대비 552만톤에 그쳤다고 보고하였고(Democratic People's Republic of Korea, 2021), 유엔식량농업기구(Food and Agriculture Organization)는 2022년 북한을 외부 식량지원이 필요한 44개국 중 하나로 선정하였다(Food and Agriculture Organization, 2020). 만성적 식량부족은 북한주민들 특히 영유아의 건강에 심각한 영향을 끼쳤을 것으로 사료된다.

영유아 시기 영양공급 부족은 여러 건강문제와 직간접적으로 연결되어 있다(Victora et al., 2008). 특히 임신 후부터 생후 2세까지 약 1,000일 동안은 성장과 발달에 매우 중요한 시기로, 이때 적절한 영양공급을 받지 못하면 면역력 저하, 성장지연 뿐 아니라 정서적 발달 장애와 지적능력 저하를 일으킬 뿐만 아니라(Schwarzenberg & Georgieff, 2018) 성인기까지 영향을 미치고 비만, 당뇨 등 만성질환 발생 위험을 높인다(De Rooij et al., 2021).

어린이 영양상태를 평가하는 방법에는 여러 가지가 있다(Table 1). 가장 손쉽게 평가할 수 있어 대규모 역학조사에서 자주 사용하는 방법은 왼쪽 팔 어깨와 팔꿈치 중간에서 팔둘레(mid-upper arm circumference)를 측정하는 것이다(World Health Organization, 2009). 또한, 세계보건기구 어린이 성장 기준(World Health Organization [WHO] child growth standard)에 따른 height for age z-score, weight for age z-score, weight for height z-score 값으로 영양상태를 평가한다(WHO Multicentre Growth Reference Study Group, 2006). 한편 WHO 어린이 성장 기준은 전세계 어린이를 위한 기준값이므로 우리나라 어린이와 청소년의 성장 및 영양상태를 정확하게 평가하는데 한계가 있다. 이에 대한소아과학회와 질병관리본부는 우리나라 어린이와 청소년의 신체계측 자료를 활용하여 2017 소아 청소년 성장도표(2017 Korean National Growth Charts)를 개발하였다(Kim et al., 2018).

2022년 5월 12일, 북한당국은 코로나19 사태가 시작된 이후 처음으로 확진자가 발생했음을 공식 인정하고 국가방역체계를 최대비상방역체계로 이행하였다(BBC news, 2022). 이미 가중된 식량난으로 인해 영양부족이 심각한데 코로나19까지 겹쳐 북한 어린이들의 건강이 심히 우려된다. 이 종설에서는 지금까지 시행된 북한어린이와 북한이탈어린이 대상 연구 결과들을 종합하여 정리하고, 또한 통일대비 모자보건 전문가들의 역할에 대해 논의하고자 한다.

본 론

1. 북한어린이 건강 및 영양상태

남북한 모자보건 주요지표(The Global Health Observatory, 2022a)를 비교하면 Table 2와 같다. 모성사망률은 출생아 10만 명당 2010년 106명에서 2017년 89명으로 감소하였다. 신생아 사망률은 출생아 1,000명당 2010년 15.9명에서 2020년 8.9명으로, 유아사망률은 23.1명에서 11.6명으로, 5세 미만 아동 사망률은 29.5명에서 16.5명으로 감소하였다. 남한과 비교하면 여전히 큰 격차를 보이지만, 그럼에도 매년 꾸준히 감소하고 있다.

북한의 아동 예방접종률과 빈혈 유병률(The Global Health Observatory, 2022b)은 Table 3과 같다. 북한 아동 예방접종률은 2010년 이후 계속 증가하여 거의 모두 97%–99%에 이르러 남한 예방접종률과 비슷한 상태이다. 하지만, 코로나19로 2년 넘게 국경을 봉쇄하면서 백신물량이 점점 부족해지고 있으며, 가장 먼저 소아마비 백신이 부족하여(KBS World, 2021) 2020년 소아마비 예방접종률이 70%까지 급감하였다. 국경봉쇄가 더 길어지면 곧 다른 백신도 부족해질 수 있으므로 이에 따른 대비가 필요하다.

5세 미만 아동 빈혈유병률은 2010년 32.5%에서 2019년 31.6%로 소폭 감소하는데 그쳤고, 15-49세 가임기 여성 빈혈유병률은 2010년 31.6%에서 2019년 33.9%로 매년 증가하고 있다.

1990년대 중반 국제사회에 북한의 식량부족 사태가 보고된 이후 시작된 식량지원 프로그램의 효과 검증을 위해 주기적으로 영양조사가 실시되었다(Central Bureau of Statistics, 1998; Central Bureau of Statistics, 2000; Central Bureau of Statistics, 2005; UNICEF & Central Bureau of Statistics, 2010; UNICEF & Central Bureau of Statistics, 2018; UNICEF et al., 2003; UNICEF et al., 2013). 1998년 조사 결과, 5세 미만 아동 영양장애 유병률은 만성영양장애 62.3%, 저체중 60.6%, 급성영양장애 15.6%였고(Central Bureau of Statistics, 1998), 이후 계속 감소되어 2017년 조사 결과 만성영양장애 19.1%, 저체중 9.3%, 급성영양장애 2.5%였다(UNICEF & Central Bureau of Statistics, 2018). 가장 최근 VNR보고서에서도 2020년 만성영양장애를 17.4%로 보고하여(Democratic People's Republic of Korea, 2021) 비록 여전히 심각한 상태이지만, 1990년대 이후 매년 조금씩 호전되고 있다(Fig. 1).

2. 북한이탈가정 어린이 영양

그동안 북한어린이 건강상태는 국제기구 영양조사를 통해 추정할 수 있었지만, 국제정세 변화와 코로나19로 인해 그마저도 불투명한 실정이다. 이런 상황에서 남한에 거주하고 있는 북한이탈가정 어린이들의 영양상태를 파악하는 것은 여러 측면에서 의미 있는 일이다. 먼저 이들을 통해 북한어린이의 건강을 간접적으로나마 유추할 수 있다. 또한 남한정착 후 북한이탈어린이들의 건강 및 영양상태 변화는 향후 국제정세가 좋아지고, 남북교류협력이 정상화되어 통일까지 고려할 때, 미래 북한어린이들의 건강과 영양상태가 어떻게 변할지 예측할 수 있는 가늠자가 될 수 있다. 하지만, 북한이탈어린이들의 건강연구는 아직까지 대부분 소규모 연구로 이루어져 전체 북한이탈어린이들의 건강상태를 파악하는데 한계가 있으므로, 추후 대규모 코호트 연구가 필요하다.

현재 남한에 입국한 북한이탈주민의 수는 2022년 3월 기준으로 33,826명이며, 이들 중 19세 이하는 5,107명으로 약 15%를 차지한다(Ministry of Unification, 2022). 북한이탈어린이 연구는 시기별로 ‘탈북 직후’, ‘남한 입국 직후’, ‘남한 정착 후’로 구분할 수 있다(Table 4). 탈북 직후에는 대상자들을 접촉하기가 거의 불가능했기에 이들의 건강과 영양 관련 연구는 매우 제한적이다. 일부 연구진이 1999년 탈북하여 중국연변에 거주하는 436명의 북한이탈어린이와 청소년을 직접 계측한 결과, 이들의 키와 몸무게는 남한 기준치 70%–90% 정도밖에 되지 않았다(Chang et al., 2000). 남한 입국 직후 연구들은 연구자들이 하나원 건강검진 자료를 이용하거나 하나원에 방문하여 직접 대상자들을 조사하였다. 2000년대 초반 입국자들 조사결과, 만성영양장애 29%-30%, 저체중 10%-28%였는데(Kim, 2005; Pak, 2004), 2000년대 후반 입국자들은 영양상태가 조금 호전되어 만성영양장애 18.6%, 저체중 14.7%였다(Lee et al., 2011). 남한 정착 후 연구들은 주로 연구자들이 지역하나센터, 대안학교, 종교시설, Nongovernmental Organization 등을 통해 대상자들을 접촉하여 조사하였다. 한 연구에서는 북한이탈 청소년들의 키와 몸무게가 남한청소년들 보다 4.9-10.8 cm 작고, 6.0-12.5 kg 적었다(Choi et al., 2010). 또 다른 연구에서는 이들의 영양상태를 만성영양장애 7.0%, 저체중 6.8%, 급성영양장애 5.3%로 보고하여 남한 입국 직후 연구 결과보다는 호전된 한편, 비만유병률이 9.1%로 증가하여 비만이 새로운 문제로 등장하였다(Kim & Choi, 2020). 대부분의 연구가 단면적 영양상태만 파악하는 일회성 조사였는데 비해, 한 연구에서는 남한 정착 2년 후 재조사하여 남한 입국 시 영양상태와 얼마나 달라졌는지 그 변화를 보고하였다(Lee et al., 2015). 6–15세 70명 대상자들의 영양상태는 2년만에 만성영양장애는 11.4%에서 5.7%로, 저체중은 14.3%에서 1.4%로 감소하여 상당히 호전되었으며 비만은 1.4%에서 5.7%로 증가하였다.

3. 모자보건 전문가들의 역할

앞서 보았듯이 북한어린이들의 건강과 영양상태는 여전히 심각한 상황이다. 북한어린이들의 건강을 위해 하루 빨리 남북모자보건 전문가들이 함께 협력할 수 있게 되길 바라지만, 교착된 남북관계는 좀처럼 풀릴 기미가 보이지 않는다. 그럼에도 평화교류의 시기는 도둑같이 임할 것을 믿고, 모자보건 전문가들이 남북교류협력이 재개될 시기를 대비해 준비할 일들을 정리해 보고자 한다.

지금과 같이 남북교류협력이 중단된 시기에는 북한어린이 건강을 위해 모자보건 전문가들이 국제사회와 우리정부에 인도적 지원을 촉구해야 한다. 또한 단순한 인도적 지원 요청만이 아니라 인도적 지원에 대한 제재완화, 면제대상 확대, 면제 관련 승인절차 간소화 등 관련 현안에 대해서도 모자보건 전문가들이 목소리를 높여야 한다. 둘째, 남북교류협력이 재개될 때를 대비하여 어느 지역, 어떤 분야를, 어떻게 모자보건전문가들이 협력할지, 모자보건학회 차원에서 구체적인 방법과 우선순위를 세워야 한다. 이를 위해선 학회차원에서 북한모자보건연구회를 구성하여 꾸준히 북한 모자보건실태를 연구하여 학회지에 발표할 수 있도록 지원해야 할 것이다. 국제기구 보고서, 북한 소아과 및 산부인과 학회지 등을 리뷰할 뿐 아니라, 로동신문 등 북한 매체를 꾸준히 분석할 필요가 있다. 이번 코로나19 상황에서 로동신문을 통해 성인뿐 아니라 임산부와 어린이용 ‘신형코로나바이러스 감염증 치료안내지도서’를 발표하였다. 셋째, 대북제재로 인해 교류협력이 어려울지라도 가능한 남북교류협력을 시도해야 한다. 대표적인 분야가 인적교류와 지식공유 사업이므로, 남북교류단체 및 시민단체들과 협력하여 남북모자보건 전문가 교류 프로그램 등을 기획하여 시도해 볼 수 있다. 이미 (사)어린이어깨동무는 평양의과대학병원 의료인 연수사업을 중국에서 실시한 바 있다. 또한 북한어린이는 접촉할 수 없을지라도 남한과 중국에 거주하고 있는 북한이탈어린이들 대상 코호트 구축 및 지속적인 모니터링을 실시할 수 있다.

결 론

1990년대 이후 조금씩 호전되어 온 북한어린이들의 건강과 영양상태는 최근 북한 코로나19 환자 급증으로 인해 다시 심각해질 위험에 처해있다. 이를 위해 모자보건 전문가들은 남한정부와 국제사회에 인도적 지원을 요청하고 여러 가지 변화상황에 대비하여 모자보건 프로그램을 준비하여야 한다.
