Trans Abstract

Purpose

To analyze the difference in bony pelvimetry using magnetic resonance imaging (MRI) in women who delivered vaginally and those who underwent cesarean section (CS) due to cephalopelvic disproportion (CPD).

Methods

This retrospective case-control study enrolled Korean women who gave birth to their first child between 37 and 42 weeks of pregnancy and underwent pelvic MRI between January 2014 and June 2020. These women were clas sified into 3 groups: vaginal delivery (VD), CS due to CPD (CPD-CS), and CS due to reasons other than CPD (Other-CS). In MRI images, true conjugate (TC), obstetric conjugate (OC), mid anterior-posterior (AP) distance (MAD), outlet AP distance (OAD), ischial spine distance (ISD), ischial tuberosity distance (ITD), and pubic angle (PA) were measured.

Results

In total, 392 women were enrolled. There were no significant differences in maternal height and body mass index at birth among the 3 groups. The ISD was statistically different between the VD group (106.2±6.4 mm) and the Other-CS group (110.4±8.8 mm) (p=0.014). After multiple regression analysis, short ISD (adjusted odds ratio [aOR], 0.92; 95% confidence interval [CI], 0.87-0.97; p=0.002), short MAD (aOR, 0.95; 95% CI, 0.89-0.99; p=0.047), and short ITD (aOR, 0.95; 95% CI, 0.91-0.99; p=0.019) were significantly associated with CS due to CPD.

Conclusion

Short ISD, MAD, and ITD, measured using MRI, are important parameters that can predict CS due to CPD. The accumulation of related research could promote the development of an excellent imaging method in terms of cost-effectiveness. Thus, it may be possible to establish a method to reduce obstetric and perinatal complications due to dystonia.

서 론

성공적인 질식분만을 위해서는 태아의 선진부의 크기 및 방향 그리고 모체 골반의 크기와 모양 등이 조화로와야 한다(Perlman et al., 2019; Toh-Adam et al., 2012). 태아 하강 중에 통로 역할을 하는 모체의 뼈골반은 입구, 중간 골반, 출구 및 치골각으로 구성된다. 아두골반 불균형은 모체의 골반 크기와 태아 두부의 크기가 맞지 않는 것이 특징이다. 아두골반 불균형이 있는 경우에 폐쇄 분만, 난산, 산도 손상 및 산후 출혈 발생 위험이 높다(Toh-Adam et al., 2012). 여러 연구에 따르면 아두골반 불균형은 모체의 키가 작은 것과 관련이 있으며(Liselele et al., 2000; Mendez-Dominguez et al., 2021; Sheiner et al., 2005; Shirley et al., 2020; Toh-Adam et al., 2012), 아두골반 불균형으로 인해 제왕절개를 받은 여성은 질식 분만을 받은 여성보다 키가 작은 것으로 보고되었다(Mendez-Dominguez et al., 2021). 아두골반 불균형이 있는 여성은 성공적인 질식분만을 한 여성보다 골반 입구, 중간 골반, 골반 출구가 모두 작은 것으로 알려져 있다(Gowri et al., 2010). 키가 큰 여성에서 골반 크기가 더 크며 아두골반 불균형 때문에 제왕절개술로 분만을 하는 경우는 드물다. X선 골반측정법은 1970년대에 도입되었지만 아두골반 불균형에 대한 예측 정확성이 낮으며, 방사선 노출에 대한 위험성이 있어 최근에는 거의 사용되지 않는다(Rozenberg, 2007). 체계적 문헌 고찰과 메타 분석을 한 연구에서 X선 골반계측법의 사용은 분만 방법을 결정하는 데 유용한 정보를 주지 못한다고 결론지었다(Pattinson, 2000; Pattinson et al., 2017).

자기공명 영상(magnetic resonance imaging, MRI) 및 컴퓨터단층촬영(computed tomography, CT)을 사용하여 아두골반 불균형을 예측하려는 여러 연구가 시도되었다. MRI로 모체의 골반 형태와 크기, 태아 머리 부피를 평가해서 난산을 예측할 수 있다는 결과도 있었지만, 아직 정확한 평가 방법이 정립되지 않았다(Gowri et al., 2010; Salk et al., 2016; Zaretsky et al., 2005).

골반중앙(midpelvis)의 지표인 양측 궁둥뼈가시(ischial spine)는 태아 머리의 모체 골반내 진입(engagement)을 평가하는 기준점이며, 태아 하강 정지는 궁둥뼈가시에서 가장 흔하게 발생한다(Gerace, 1959). 그러나 궁둥뼈가시거리는 검사자의 손으로 주관적으로 측정하기 때문에 신뢰도가 떨어지므로(Perlman et al., 2019), 이에 대한 정확한 평가가 필요하다. 또한, 이전의 연구에는 한국을 포함한 아시아계 여성이 거의 포함되지 않았다.

이 연구의 목적은 한국 여성에서 MRI를 기반으로 측정한 골반입구(pelvic inlet), 골반중앙(midpelvis), 골반출구(pelvic outlet)의 직경, 두덩결합(symphysis pubis) 각도 등이 제왕절개분만과 관계가 있는지 보기 위함이다.

대상 및 방법

이 연구는 후향적 환자-대조군 연구로, 이전에 분당차병원에서 임신 중 합병증을 진단받지 않은 상태로 임신 37주 이상 42주 미만에 질식분만 혹은 제왕절개분만으로 초산을 한 뒤, 2014년 1월부터 2020년 6월까지 분당차병원에서 근종, 자궁선근증, 부인암 등 부인과 질환 진단을 위하여 골반 MRI 촬영을 시행한 한국 여성을 대상으로 하였다. MRI 촬영 당시의 나이가 20세 이상 50세 미만이면서, 선천적 골반 기형이 없고 골반 외상 병력이 없는 여성을 포함했다. 이들 여성을 VD군(질식분만 군), CPD-CS군(아두골반불균형으로 인한 제왕절개 분만군), Other-CS군(아두골반불균형 외의 이유로 제왕절개 분만한 군) 중 하나로 분류했다. 이 연구에서 아두골반불균형은 분만 진행이 지연되는 지연장애(protraction disorder) 혹은 분만 진행이 중단된 정지장애(arrest disorder)가 진단된 경우로 정하였다(Nelson et al., 2020). 여성의 키, 체중, 체질량지수(body mass index, BMI) 등에 관한 임상 정보는 의무 기록지를 통하여 수집하였다.

골반 MRI는 앙와위 자세에서 촬영되었으며, 세 가지 MRI 시스템(SIGNA 3.0T HDXT, General Electric Co., Boston, MA, US, 2006; SIGNA ARCHITEC, General Electric Co., 2018; SIGNA HDX 1.5T, General Electric Co., 2006) 중 하나를 사용하였다. MRI 영상에서 골반 입구 크기를 반영하는 참앞뒤지름(true conjugate, TC)과 출산앞뒤지름(obstetric conjugate, OC), 중간 골반 크기를 반영하는 중간골반앞뒤거리(mid anterior-posterior [AP] distance, MAD), 골반 출구 크기를 반영하는 골반출구앞뒤거리(outlet AP distance, OAD), 궁둥뼈가시거리(ischial spine distance, ISD), 궁둥뼈결절거리(ischial tuberosity distance, ITD)를 측정하였으며, 두덩결합 각도(pubic angle, PA)를 측정했다(Fig. 1).

TC는 엉치뼈곶(sacral promontory)에서 두덩결합(pubic symphysis)의 상단까지로 측정했으며. OC는 엉치뼈곶에서 두덩결합의 안쪽 가장자리까지로 측정했다. MAD는 엉치뼈의 S4와 S5 사이의 지점에서 두덩결합 아래쪽 가장자리까지로 측정했다. OAD는 엉치뼈의 끝에서 두덩결합의 안쪽 피질 기저부까지로 측정했다. ISD는 궁둥뼈가시 사이의 가장 짧은 거리로 측정했으며, ITD는 궁둥뼈결절 사이의 가장 넓은 거리로 측정했다. PA는 양쪽 궁둥뼈결절과 두덩결합 하부의 안쪽 가장자리 사이의 각도로 하였다.

통계 분석을 위하여 IBM SPSS Statistics ver. 22.0 (IBM Co., Armonk, NY, USA)을 이용하였다. 세 그룹 간의 비교에서 연속 변수는 일원 분산 분석과 함께 Tukey honestly significant difference (HSD) 검정을 사용했다. 분만 당시 여성의 연령, 키와 BMI, 분만 주수, 신생아 출생시 몸무게, MRI 촬영 당시의 연령, 분만과 MRI 촬영 사이의 시간 간격을 포함한 다중회귀분석을 수행했다. 모체의 키와 아두골반 불균형으로 인한 제왕절개분만 사이의 상관관계를 보기 위하여 피어슨 상관 계수를 구하였다. 통계적 유의성은 p<0.05인 경우로 하였다.

이 연구는 기관심의위원회와 분당차병원 윤리위원회(No. 2020-06-003)의 승인을 받았다. 후향적 의무기록 분석 연구로, 연구 대상자의 동의서 획득은 면제되었다.

결 과

총 392명의 여성이 포함되으며, VD군은 258명, CPD-CS 군은 34명, Other-CS군은 100명이었다. 이 세 그룹에서 분만 당시 나이, 분만 당시 키와 BMI, 분만 주수, 신생아 출생 시 몸무게는 통계학적으로 유의한 차이가 없었다. MRI 촬영 당시 나이는 VD군 44.1±1.9세, CPD-CS군 44.7±2.0세, Other-CS군 44.7±1.6세로 통계학적으로 유의하게 차이가 있었다(p=0.014). 분만과 MRI 촬영 사이의 시간 간격 또한 각각 14.2±3.5년, 16.0±5.2년, 15.4±0.4년으로 통계학적으로 유의한 차이가 있었다(p=0.003) (Table 1).

세 군에서 MRI에서 얻은 골반계측값을 비교한 결과, TC, OC, MAD, OAD, ITD 및 PA는 통계학적으로 유의한 차이는 없었으나 ISD의 경우에는 VD군 108.7±8.2 mm, CPD-CS군 106.2±6.4 mm, Other-CS군 110.4±8.8 mm로 통계학적으로 유의하게 차이가 있었다(p=0.028) (Table 2). Tukey HSD test 을 시행했을 때 VD군과 CPD-CS군 사이에 ISD의 통계학적으로 유의한 차이는 없었지만, 두 군의 기본적인 임상적 특징 차이가 있었기 때문에 분만 당시 나이, 분만 당시 키와 BMI, 분만 주수, 신생아 출생 시 몸무게, MRI 촬영 당시 나이, 분만과 MRI 촬영 사이의 시간 간격 변수를 포함하여 다중회귀 분석을 시행했다. 그 결과, 짧은 ISD는 통계적으로 유의하게 아두골반 불균형으로 인한 제왕절개분만과 관련이 있었다(adjusted odds ratio [aOR], 0.92; 95% confidence interval [CI], 0.87-0.97; p=0.002) (Table 3). 더불어 짧은 MAD (aOR, 0.95; 95% CI, 0.89-0.99; p=0.047) 및 짧은 ITD (aOR, 0.95; 95% CI, 0.91-0.99; p=0.019)도 다중회귀 분석 결과 통계적으로 유의하게 아두골반 불균형으로 인한 제왕절개분만과 관련이 있었다.

한편 모체의 키와 아두골반 불균형으로 인한 제왕절개분만은 통계학적으로 상관관계가 없었다(Pearson r=0.082, p=0.161).

고 찰

이 연구는 한국여성을 대상으로 MRI로 평가한 골반 크기와 아두골반 불균형으로 인한 제왕절개분만과의 관련성을 분석한 최초의 연구이다. 우리는 MRI를 통해 측정한 골반계측 변수 중에 궁둥뼈가시거리인 ISD의 단축이 아두골반 불균형을 가장 잘 반영함을 보여주었다. 또한 중앙 골반 크기를 반영하는 중간골반앞뒤거리 MAD와 골반 출구 크기를 반영하는 궁둥뼈결절거리인 ITD가 짧은 것도 아두골반 불균형으로 인한 제왕절개 분만과 관계가 있음을 보여주었다.

성공적인 질식분만에는 여성의 연령, BMI, 태아의 크기, 태향, 태축, 여성의 골반 크기 등과 같은 여러 요인에 의해 영향을 받는다(Gowri et al., 2010; Korhonen et al., 2015; Maharaj, 2010). 태아의 크기, 태향, 태축은 분만 전과 분만 중에 객관화된 검사인 초음파로 확인할 수 있지만, 모체의 골반 크기 측정은 주관적인 내진으로 평가되기 때문에 신뢰도가 낮다. 따라서 아두골반 불균형을 예측하는 방법에 대한 여러 연구가 지금까지 이뤄지고 있다.

이전 여러 연구에서 모체의 키와 아두골반 불균형 사이의 관계를 평가했으며, 여성의 작은 키는 아두골반 불균형의 위험도를 높임을 보여주었다(Oboro et al., 2010; Perlman et al., 2019; Rozenholc et al., 2007; Sheiner et al., 2005; Toh-Adam et al., 2012). 그러나 이 연구에서는 모체의 키와 아두골반 불균형으로 인한 제왕절개분만 사이의 통계학적으로 유의한 상관관계는 볼 수 없었다.

다른 연구에서는 태아의 머리 크기와 모체의 골반 크기 비율을 구함으로써 아두골반 불균형을 예측하기도 했다(Fasler et al., 2010; Mendez-Dominguez et al., 2021; Zaretsky et al., 2005). 그러나 태아의 머리 크기는 임신 주수가 증가함에 따라 변하므로 분만예정일 한참 전에는 미리 아두골반 불균형을 예측할 수 없는 단점이 있다.

어떤 연구에서는 X선, CT를 이용하여 모체의 골반계측을 시도하였다(Chen et al., 1982; Martinsen et al., 2005; Salk et al., 2016). 그러나 X선, CT 촬영을 위해서는 방사선 노출을 감수해야 하는 단점이 있다.

Klemt 등(2019)은 성공적인 둔위 질식분만을 위해 MRI를 사용하여 여성의 골반 크기를 측정했으며, MRI를 이용한 골반 계측이 둔위에서 질식분만 성공을 예측할 수 있는 가능성을 보여주었다. 그러나 다양한 인종 특성을 반영하지 못한 한계가 있다.

우리 연구에서 MRI로 측정한 짧은 ISD는 아두골반 불균형으로 인한 제왕절개분만과 통계학적으로 유의하게 관련이 있었다. ISD는 뼈골반(bony pelvis)과 태아 머리의 진입(engagement)을 평가하는 기준점 사이의 최단 거리인데 (Aronson & Kier, 1991), 내진으로 직접적인 측정은 어렵다. 궁둥뼈가시가 현저히 튀어나와 있음이 느껴지면 골반 협착을 의심할 수는 있지만 손으로 정확하게 거리를 측정하기 힘들다. 한편 이 연구에서 아두골반 불균형으로 인한 제왕절개분만과 관련 있었던 MAD는 중앙 골반 크기를 반영하므로 손으로 측정하는 것은 불가능한데, MRI에서는 어렵지 않게 계측할 수 있는 지표이다.

이 연구의 강점은 한국 여성만을 대상으로 시행한 MRI로 평가한 골반 크기와 아두골반 불균형으로 인한 제왕절개분만과의 관련성을 분석한 최초의 연구라는 점이다. 인종마다 골반을 포함한 체형이 다르므로, 아두골반 불균형을 정확하게 평가하는 기준을 세우기 위해서는 인종에 특화된 연구가 필요하다.

그러나 이 연구에는 한계점이 있다. 첫 번째는 후향적 의무기록 분석 연구라는 한계점 때문에 분만 직전 태아 및 분만직후 신생아의 머리크기를 모체 골반계측값과 함께 분석하지 못했다는 한계가 있다. 두 번째는 MRI 촬영이 분만 후에 이뤄졌으므로, MRI가 분만전의 골반 상태를 온전하게 반영하지 못했을 가능성이 있다. 이 점을 보완하기 위하여 이 연구자들은 다른 이유로 제왕절개분만을 한 군을 연구에 추가하여 질식분만을 한 군과 골반 측정 변수들을 비교하여 분석했으며, 이 두 군 사이에는 통계적으로 유의한 차이가 없음을 확인하였다. 그러나 추후에는 임신 전 MRI 촬영 결과와 이후의 분만방법 사이의 관계를 비교하는 연구가 필요하다. 세 번째는 분석 대상의 수가 많지 않아 아두골반 불균형을 예측할 수 있는 ISD, MAD, ITD의 기준값을 설정할 수 없었다. 향후에 이를 위한 대규모 전향적 연구가 필요하다.

결 론

결론적으로 우리는 MRI에서 계측한 짧은 ISD, MAD, ITD 는 아두골반 불균형으로 인한 제왕절개분만을 예측할 수 있는 중요한 골반 측정 변수임을 보여주었다. 아두골반 불균형이 의심되지만 골반 내진으로 확실한 진단을 내리기 어려운 경우에는 MRI를 이용한 골반계측이 도움이 될 수도 있다. 아직 비용효과분석 면에서 MRI가 뛰어난 검사 방법은 아닐 수 있지만, 이와 관련된 연구가 쌓이면 비용효과분석 면에서도 뛰어난 영상 진단 방법을 개발하는 데 도움이 될 수 있으며, 궁극적으로 진통 전 분만 방법을 미리 결정하여 난산으로 인한 산과적 합병증 및 주산기 합병증을 줄이는 방법을 세울 수 있을 것이다.
