Trans Abstract

Purpose

To compare pregnancy complications between the 2 groups of patients with gestational diabetes mellitus (GDM): those diagnosed by the 1-step method and those diagnosed by the 2-step method.

Methods

In this retrospective cohort study, the data from outpatient and hospitalization medical records of 201 patients diagnosed with GDM between 2013 and 2017 were reviewed. We compared the pregnancy complications of these patients based on whether they were diagnosed by the 1-step or 2-step method. SPSS ver. 20.0 was used to analyze the data from the 2 groups. The odds ratio and 95% confidence int-erval of the pregnancy outcomes were estimated using binary logistic regression analysis.

Results

On comparing pregnancy-related complications between the groups, there was no significant dif ference in the incidence of preeclampsia or delivery by cesarean section (p>0.99 and p=0.50, respectively). In the 1-step and 2-step groups, the prevalence of premature birth was significantly high at 19.7 % and 40.3% (p=0.01), respectively. There were no significant differences between the 2 groups in terms of macrosomia, large for gestational age (LGA), small for gestational age (SGA), low APGAR score, and neonatal hypoglycemia (p>0.99, p>0.26, p>0.62, p>0.57, and p>0.45, respectively).

Conclusion

On comparing the 2 groups, we found that the 1-step and 2-step GDM groups had similar risks of pregnancy complications, namely preeclampsia, delivery by cesarean section, macrosomia, LGA, SGA, low APGAR scores, and neonatal hypoglycemia.

서 론

임신성 당뇨병은 임신으로 인한 생리적 변화에 의해 임신 중에 발생 또는 임신 중 처음 발견된 내당능 장애로 정의하고 있다(Caissutti & Berghella, 2017). 임신 중 모체가 태아에게 적절한 영양공급을 하기 위해서는 당 대사에 변화가 생기게 되는데 이는 공복 시 저혈당증, 고인슐린혈증, 그리고 식후 고혈당증이 나타나게 되고, 이러한 변화로 인슐린의 민감도가 떨어지기 시작하면서 인슐린의 저항성이 증가하게 되어 임신성 당뇨가 발생하게 된다. 산모의 고혈당은 태아의 고인슐린혈증을 유발하게 되어 지나친 신체성장으로 인한 거대아를 유발한다. 이러한 임신성 당뇨병의 위험 요인으로는 비만, 당뇨의 가족력, 이전의 임신성 당뇨, 내당능장애, 거대아 분만력, 당뇨 등이 있다. 임신성 당뇨로 인해서 발생할 수 있는 단기적 합병증으로는 임신성 고혈압 및 전자간증 및 신생아적 합병증으로는 거대아, 부당 중량아 그 외 견갑난산, 조산, 제왕절개로의 분만, 수술적 합병증, 산모 출혈, 감염을 유발할 수 있으며 장기적으로는 제 2형 당뇨병, 심혈관계질환, 대사증후군, 비만을 유발할 수 있다(Benhalima et al., 2015).

임신성 당뇨의 합병증을 예방하기 위해 임신성 당뇨병을 적절한 기준과 방법으로 진단하기 위한 선별검사 및 진단검사는 다양한 연구를 거쳐 그 기준과 방법이 발전되어 왔으나 이에 대해서는 세계적으로 일치된 기준이 없다(Farrar et al., 2017). 최근에 여러 기관에서 선별검사 및 진단검사의 방법과 기준에 대한 논의가 지속적으로 이루어지고 있는 부분 중, 특히 국내에서는 2단계 검사법과 1단계 검사법이 가장 많이 사용되고 있다. 2단계 검사는 금식 없이 50 g의 당을 복용하고 1시간 후 혈중 당농도를 측정하여 130 또는 140 mg/dL 이상인 군을 선별하고, 진단검사로 8시간의 금식 후 100 g의 당을 복용 후 1시간, 2시간, 3시간의 혈중 당농도를 측정하여 기준치의 2개 이상일 때 임신성 당뇨를 진단하는 Carpenter-Coustan (CC) 기준 가장 많이 사용하고 있다. 2008년도에 임신중 고혈당과 임신 합병증의 관계를 7년간 전향적으로 관찰한 다기관 공동연구 Hypoglycemia and Adverse Pregnancy Outcomes study에서 임신 중 정상으로 여겨지는 혈당 수치에서 조차도 혈당에 비례하여 과체중아, 제왕절개율, 신생아 저혈당을 포함한 임신 합병증이 증가한다는 결과를 제시하여 2010년도부터 International Association of Diabetes and Pregnancy Study Group (IADPSG)에서는 선별 및 진단검사로서 금식 후 75 g 당 복용 후 식후 1시간, 2시간 혈당을 측정하는 진단 기준을 제시하였다(Metzger et al., 2008; Metzger et al., 2010).

임신성 당뇨병은 전세계적으로 5.8%–12.9%로 보고되며(Zhu & Zhang, 2016), 우리나라는 1995년도에는 2%의 유병률을 보였고 2011년도에는 9.5%로 국내는 물론 전 세계적으로 임신성 당뇨병은 증가하고 있는 추세에 있다(Kang et al., 2010; Koo et al., 2016). 이러한 유병률의 증가는 고령임신, 다태임신 및 비만 임신부의 증가와 같은 위험 요인의 증가에 따른 것으로 보이며 이러한 상황에서 적절한 선별 및 확진 검사의 기준과 방법에 대한 고찰이 필요하여(Ferrara, 2007) 2단계 접근법과 1단계 접근법의 신생아 아프가 점수, 부당중량아와 부당경량아, 신생아 저혈당 및 조산을 비교한 연구는 없어 이에 대해 연구하고자 하였다(Farrar et al., 2017).

대상 및 방법

이 연구는 후향적 코호트 연구로 2013년 1월부터 2017년 12월까지 5년간 이대목동병원에서 임신성 당뇨로 진단받은 201명이 대상으로 외래 및 입원 의무 기록의 자료를 검토하였다. 이 중 다태임신 및 자료가 불충분하거나 기존의 1형 또는 2형으로 진단받은 과거력이 있는 임신부를 제외한 155명이 연구 대상자로 선정되었다. 임신 24주에서 28주 사이에 검사가 진행되었고, 75 g 경구당부하 검사만으로 진행된 1단계 검사법으로 진단받은 환자는 77명이었고 8시간 금식 후 공복혈당 당부하후 1시간, 2시간 세 번의 채혈 중 92, 180, 153 mg/dL 중 이상 소견이 있을 시 진단하는 IADPSG 기준에 따라 진단하였다. 50 g 경구당부하검사로 선별검사 후 140 mg/dL인 경우에 100 g 경구당부하검사로 확진하는 2단계 검사법으로 공복혈당, 1시간, 2시간, 3시간 채혈 후 진단 기준 95, 180, 155, 140 mg/dL 로 2개 이상이 높을 시 CC 기준으로 진단받은 임신부는 78명을 포함하였다(Fig. 1).

산모의 특성으로는 나이, 임신 전 체질량지수, 고혈압 및 당뇨의 과거력, 고혈압 및 당뇨의 가족력이 수집되었고 임신 기간 동안 체중 증가량, 인슐린 요법을 받았는지 여부, 당화혈색소 수치를 검토하였으며 분만 주수, 임신 합병증 여부, 신생아의 거대아 여부, 부당중량아, 부당경량아 및 아프가 점수, 신생아 저혈당증의 발생율을 비교하였다.

두 집단의 비교 결과는 IBM SPSS Statistics ver. 20.0 (IBM Co., Armonk, NY, USA)을 이용하였으며 임신부의 특성 및 임신 결과에 대하여 연속 변수에 대해서는 평균±표준편차로 표기 및 Student t-test를 이용하여 비교하였고, 범주형 변수에 대해서는 빈도(백분율)로 표기, chi-square test를 통해 분석하였으며 두 집단의 임신 결과에 대한 분석 방법도 동일하였다. 두 그룹을 이분형 로지스틱 회귀 분석을 통하여 임신 결과의 교차비(odds ratio) 및 95% 신뢰구간(confidence interval)을 추정했으며, 통계학적 유의성은 p<0.05로 정의하였다.

결 과

두 그룹 간의 임상 특성을 비교하였는데 산모 나이, 임신 전 체질량지수, 고혈압의 과거력, 당뇨와 고혈압의 가족력 은 p 값 0.39, 0.90, 0.99 이상, 0.40, 0.99 이상으로 각각 유의한 차이가 없었고, 임신 중 체중의 변화와 인슐린 치료 여부도 p 값 0.09, 0.41로 유의한 차이가 없었다. 선별검사 당시 시행한 당화혈색소 수치는 IADPSG 그룹에 비해 CC 그룹이 p 값 0.01로 유의하게 높았다(Table 1).

두 그룹 간의 임신관련 합병증에 대해 비교하였을 때 전자간증, 제왕절개수술로의 분만은 p 값 0.99 이상, 0.50로 두 집단 간의 유의한 차이는 없었으며, CC 기준으로 진단받은 군에서 조산으로 인한 분만이 40.3%로 IADPSG 기준으로 진단받은 군의 19.7%와 비교하였을 때 p 값 0.01로 통계적으로 유의한 차이를 보였다. 조산으로 분만한 군 중 32주 미만의 분만은 1단계 검사법으로 진단받은 그룹이 3명으로 3.9%, 2단계 검사법으로 진단받은 그룹이 7명으로 10.1%로 이 두 집단의 차이는 p 값 0.19로 통계적으로 분포의 유의한 차이가 없었다. 조산의 원인은 조기진통, 조기양막파열, 자간전증, 태반조기박리, 헬프증후군이 있었다. 4,000 g 이상의 거대아 분만 및 부당중량아, 부당경량아, 6점 이하의 낮은 아프가 점수, 신생아 저혈당 등은 p 값 0.99 이상, 0.26, 0.62, 0.57, 0.45으로 두 그룹간의 유의한 차이는 없었다(Table 2). 1단계 검사법에 의한 그룹에 비하여 2단계 검사법에 의한 그룹의 교차비를 분석한 결과 첫 제왕절개술에 의한 분만은 교차비 0.783으로 이를 제외한 자간전증, 조산, 거대아분만, 부당중량아, 부당경량아, 낮은 아프가점수의 교차비는 1.15, 2.75, 1.14, 2.20, 1.31, 1.58, 1.63으로 증가하는 경향을 보였으나 조산의 경우만 p 값 0.01로 통계적으로 유의한 차이를 나타냈다(Table 3).

고 찰

이 후향적 코호트 연구는 임신성 당뇨의 선별 및 진단 검사로서의 1단계 검사법인 75 g 경구당부하검사로 IADPSG 진단 기준에 따르는 방법과 2단계 검사법인 50 g 포도당 부하검사로 선별 후 100 g 경구 당부하검사로 CC 진단 기준으로 확진하는 방법을 통해 진단받은 임신성 당뇨 환자의 임신 합병증을 비교 분석하였다. 두 그룹의 나이, 임신 전 체질량지수, 고혈압의 과거력, 당뇨 및 고혈압의 가족력, 임신 중 체중의 증가량에는 통계적으로 유의한 차이가 없어 두 집단의 임신 합병증 결과는 단변량 분석을 시행하였다. 이전 문헌에는 두 집단의 진단 기준에 의한 차이로 기존의 5%–6%의 유병률이 IADPSG 진단 기준에 의하면 17.8%로 상승함을 보여 IADPSG 진단 기준에서는 이전에 진단 되지 않았던 경한 임신성 당뇨군이 더 포함되어 있을 가능성을 제시하였는데(Abebe et al., 2017; Luewan et al., 2018; Nicolosi et al., 2020; Sacks et al., 2012), 이 연구에서는 진단 당시 측정된 당화혈색소 (glycosylated hemoglobin)가 CC 기준의 2단계 검사로 진단 받은 집단에서 통계적으로 더 상승한 유의한 값을 보여 1단계 검사법으로 진단받은 집단이 더 경한 임신성 당뇨 환자군을 포함하고 있음을 간접적으로 반영하고 있었다.

두 그룹 간의 임신성 당뇨병과 관련하여 발생할 수 있는 합병증에 대해 비교한 결과 통계적으로 유의하게 차이가 있는 합병증은 조산이었으며, 그 외에 전자간증, 제왕절개술로의 분만, 거대아 분만 및 부당중량아, 부당경량아, 낮은 아프가 점수, 신생아 저혈당 등은 통계적으로 유의하지 않았고 비슷한 발생률을 보였다. 경한 임신성 당뇨병을 포함한 IADPSG 기준으로 진단받은 군도 CC 기준의 임신성 당뇨병과 유사한 비율로 임신성 당뇨병에 의한 합병증이 발생함을 보여줌으로써 이전의 연구 결과와 마찬가지로 경한 임신성 당뇨병 환자에서도 전자간증, 제왕절개술로의 분만, 거대아 분만 및 부당중량아, 부당경량아, 낮은 아프가 점수, 신생아 저혈당이 유의하게 나타날 수 있음을 알 수 있으며, 기존의 2단계 검사에서는 음성이었으나 1단계 검사로 진단받았던 산모에게서 임신성 고혈압, 전자간증, 부당중량아의 발생이 증가함을 보였던 결과를 반영했다(Caissutti et al., 2018). 마찬가지로 경한 임신성 당뇨임에도 불구하고 추적 관찰 중 식단 및 운동으로 임신성 당뇨의 목표 혈당치인 공복 95 mg/dL 미만 및 식후 2시간 120 mg/dL 미만으로 조절되지 않아 인슐린 요법을 요하는 임신부도 두 그룹 간에 유의한 차이를 보이지 않았다는 점도 이 결과를 뒷받침하고 있다.

이 연구는 국내에서 1단계 검사법과 2단계 검사법의 주산기 예후를 비교 분석한 첫 연구로서, 선별검사로 75 g 경구당부하검사를 시행하여 진단까지 시행한 집단에 대해 분석하였다는 강점이 있다. 한계점으로는 후향적 코호트 연구로 연구 자료의 제한으로 임신성 요인의 위험 요인인 이전의 거대아 분만력, 이전의 임신성 당뇨 과거력 여부에 대해서는 고려되지 않았다는 점이 있으며 임신성 당뇨 선별검사에서 정상으로 나왔던 군에 대한 비교가 되지 않았다는 점이 있다.

위 연구는 1단계 검사법으로 진단받은 임신성 당뇨 집단이 2단계 검사법으로 진단받은 임신 성 당뇨 임신부와 유사한 임신성 합병증이 있음을 보여주었다. 이는 1단계 검사가 선별 및 진단 검사로 사용될 수 있음을 시사한다. 이전의 두 검사법에 대한 비용을 비교하였을 때 2단계 검사법이 1단계 검사법에 비해 비용적인 부분에서 더 저렴할 것으로 보고된 바 있으나(Meltzer et al., 2010), 이는 임신성 합병증의 발생으로 인해 발생하는 2차적인 비용 등에 대해서는 고려되지 않은 부분이 있어 이를 고려한 비용 대비 효과에 대한 고찰이 필요할 것으로 생각된다. 1단계 검사법을 위해서는 모든 임신부가 선별검사를 위해 금식을 해야 하는 불편감이 있을 수 있겠으나, 효율적인 면에서 두 번의 검사가 아닌 한번의 검사로 진단을 할 수 있는 장점 및 임신성 당뇨 진단 결과를 더 빨리 제공할 수 있는 장점이 있고, 또한 100 g 포도당의 섭취의 불편감 및 3시간 후까지 금식상태를 유지하며 채혈해야 하는 불편감을 줄일 수 있는 장점이 있을 수 있겠다(Saccone et al., 2019). 또한 최근의 메타분석 및 무작위 대조시험에서 1단계 검사법과 2단계 검사법으로 인한 임신성 당뇨의 발생률이 큰 차이가 없음을 보여, 선행 요인에 대한 적절한 검토 후에 1단계 검사법을 적용하는 경우에는 효율적이지 않은 발병률만을 높이는 결과를 가져오지는 않을 것으로 생각할 수 있겠다(Khalifeh et al., 2020; Saccone et al., 2019).

결 론

이 연구를 통하여 IADPSG 기준의 1단계로 선별 및 진단을 하는 검사법과 CC 기준으로 2단계의 선별검사에 이은 진단검사로 진행하는 것과 비교하였을 때, 경한 임신성 당뇨를 포함한 1단계 검사법 확진군에서 유사한 임신성 합병증의 결과를 보였다. 그리하여 1단계 방법을 통한 임신성 당뇨의 진단 및 적절한 치료를 시행하여 조산, 전자간증, 제왕절개술로의 분만, 거대아 분만 및 부당중량아, 부당경량아, 낮은 아프가 점수, 신생아 저혈당을 낮추기 위한 선별 및 진단 검사로 사용될 수 있겠다. 그럼에도 불구하고 후향적 코호트 연구라는 한계점이 있으므로, 한국인을 대상으로 하는 다기관의 대규모 전향적 코호트 연구를 통하여 이에 대한 추후의 연구가 필요할 것으로 생각한다.
